Telling paradise is a very nice facility,food is fair ,everyone is helpful .ny cna Rosemarie comes in ny room with a very pleasant smile ,makes me feel very comfortable in her care.this facility is very clean house keeping does a very good job.nurse Karina is very helpful and will answer call lights if needed very caring Girl.

Our mom has been a resident at Trellis Paradise for several weeks now, and the care provided by her CNA, Dominic, and her nurse, Jeffrey, has been absolutely exceptional. Every question or concern we raise is handled with prompt, thorough attention. We are deeply grateful to Jeffrey and Dominic for the profound compassion and professionalism they continue to show our entire family.

I wrote a review about this horrible place within the last 7-8 months. Because the review was so bad, it has been eliminated from the “review” section. Please, if you care about your loved one, do not under any circumstances, ever send them to this place.

First things first, the nurse practitioner and doctors at Trellis Rehab are amazing. Nurses Jeff, Jerrick, Alexis, and Daniel have been wonderful and have stayed on top of all health issues. CNAs Kylah, Marcus, Dom, and Melanie have been so helpful. I want to send special thanks to these helpful individuals who’ve made my stay at Trellis Rehab so much better. This includes all the rehab techs who helped me walk again. I was paralyzed from the waist down; now I’m able to walk with my walker. Thank you to the case manager and the assistant case manager for their help. Thank you to Jet, the weekend nursing supervisor, who helped me with a rude agency nurse who wouldn’t give me my medication. Thank you to the kitchen assistant supervisor and supervisor for helping with the food I was being served. To the ones, I can’t remember you know who you are thank you. Vicky Wiseman

Fainting Tremors Headache Vomiting Dizziness Memory Fog aggressive Neurological Disorder Fatigued aggressive Itching aggressive Scalp Entire Body f Covered in Mold and mildew and mouth eyes! I’ve passed out 3 Times hitting my head straight down on floor. I arrived at Henderson Hospital seeing I couldn’t see well or balance well and passing out Trellis Paradise has has saved me from Death. The case manager and Social Worker are working directly with me for housing & sending a day letter now I need a lawyer! You’re entitled to receive all rent money returned due to pre-existing conditions. IM TRULY IN GOOD HANDS FOR COMPLETE RESTORATION TRELLIS PARADISE THANK YOU SO SO SO MUCH !

This place was wonderful in helping my family. The therapy team was amazing, and I especially want to shout out Shanise in Social Services. She made our whole family feel comfortable and supported throughout our loved one’s care.

This place is a nightmare in broad day light your cna need more care lesson for the patient especially Sarah who like to clean the patient with those brown paper towels and Kaya who wash a spoon with no soap and you still smell the person breath on that spoon an d last night my sister need help and the cna help her and when he left he took all the cover off of her and I had to call him back to cover her up

I stayed at Trellis Paradise from May 13, 2026 to June 26, 2026, and want to thank the nurses , CNAs, OTs, PTs and other staff for the care that I received. I especially am grateful to Nurses Jerrick-best IV nurse, Alise and Jeff who had to take and put in new IVs on my arms and very professional. Nurse Jerrick is firm and made sure I worked with the PTs and OTs. PTs and OTs helped me develop skills and my muscles as a BKA. CNAs Sandra – efficient and nice, Irene- watches and checks on patients, Dominic- efficient and friendly, Gorgeous-encouraging and inspiring words, Gwendolyn, Amelia, Rose Cruz – bubbly, and Jaeleen and the other nurses who were proficient and pleasant. APRN Jessica was friendly and professional. My family had pleasant visits with me and we enjoyed the dining area and courtyard. Thank you for helping in my recovery and assisting with my transition to home care.

I really enjoyed my stay here with the exception of your food. You really need a better cook!. I enjoyed my CNA’ : DOMNIC (aka- DOM) CNA ,HE WAS VERY NICE, PERFECTLY CARING WITH ALL PATIENCE from what I witnessed with other patients. He really has a Heart and he shows that he LOVES HIS WORK. I also have to give credit to the nurse: Jerick( day shift) he is very attentive and also shows he cares for his patient’s and I Love his sense of HUMOR.. I also want to give Daniel-RN.a thumbs up also. He always greets me with a smile everyday makes me feel happy to see me, I also, give Kyla CNA A big thumbs up. She is so sweet and very attentive, professional. and cares for her patient’s. Loved my OT, & PT THERAPISTS they were GREAT AND PROFESSIONAL and great sense of Humor.

The CNA’s and most of the nurses do not care and will leave you for hours after you have requested help several times. The food is not edible. This was the most horrible experience I have had in my life. Do not send your loved ones here to suffer!

Paradise facility is a clean, well-lit Post-Acute Care Rehabilitation Center. Much gratitude to the O.T.s, P.T.s, C.N.A.s, R.N.s that look after my loved one. I see it I the staff’s continence. I see the compassion. I hear their voice. Care. Reception always greets me with a warm welcome. Engaging visitors with compassion and professionalism. …and the star the fell from the sky, right into the reception chair, Brandy. Thank you Brandy for your encouragement and kind words. Having a loved one in a facility such as Trellis is comforting . Caring and vigilante are the O.T.s, P..T.s, C.NA.s, R.N.s, Administrators. All of the team members @ the reception/check-in are welcoming, informative, and courteous. Dominic Hassans my best CNA during my stay in the past one and a half month. He is very responsive to my needs and a very respectful gentleman.

New facility, okay nursing, horrible cna’s, food is uneatable and the beds are the cheapest most uncomfortable I’ve ever slept in! I would not recommend this nursing home to anyone!
