New facility, okay nursing, horrible cna’s, food is uneatable and the beds are the cheapest most uncomfortable I’ve ever slept in! I would not recommend this nursing home to anyone!

My brother stayed at Trellis Paradise from 2/25 to 4/8,2026. He has a long history of Parkinson’s Disease with dementia. I am his POA. Before his discharge, I accidentally found out the care manager had tried to forge my signature on his legal billing documents and even filled it out. So I went to see the manager of the facility (Andrew Coons). The nurse manager (Israel Sanchez) was also present in the office. He tried to defend their wrongdoings by saying that my brother had tried to sign the above legal documents. He even brought the social workers and two case managers to see my brother to prove that my brother was able to sign the documents. My brother lacked decision making capacity, as he is severely mentally impaired and with dementia. How can they even try to take his words or even have him sign for his POA? The manager (Andrew Coons) was not happy and became extremely rude and adamant in requesting that I showed him my POA letter. He even told me that unless I am able to provide documentation that I should leave his office immediately. As a matter of fact, my letter stating I was POA was submitted on the first day of my brother’s admission to Trellis Paradise. The care manager has always been trying to push my brother to go to the group home and hospice, which was run by her friend (Christian). That is a conflict of interest, as she is reaping the benefits by referring to her friend’s business. They even stated if my brother joined hospice care, he would get more benefits, even though there was (and still no indication to this day) for hospice care at all. Therefore she sent her friend (Christian) to evaluate my brother’s physical conditions without letting me know ahead of time. I. Zen, MD, FACC.

Grandmother was there for approx. 1 month, maybe a little more. Everyone at the front desk area was great, even the Physical therapy nurse (Angie I believe her name was) was fantastic, my grandmother’s CNA Andy was also very kind towards my grandmother and she adored him, the place is also very clean looking and welcoming, that aspect gets 5 stars. My complaint is mainly with the staff that works at the nursing stations near the dining room areas and the rooms, they always seemed to be bothered by any questions that were asked, even when I asked to borrow a pen because the one I had wasn’t working, I got an eye roll. Note that this place does not have phones in the rooms for the patients, they only have 1 phone that they use for ALL the patients and it gets lost constantly. My family members would call to talk to my grandmother and 9/10 times they couldn’t get through because they lost the phone. It was always the same “we’re working on that issue” yet never any progress seemed to be made, a simple sign out sheet of which room number you’re taking the phone would be better than just crossing your fingers and hoping you get the phone back. From what I could see there aren’t enough CNA’s/nurses to the number of patients therefore my grandmother would go unchanged for sometimes as long as 3 hours. While we were there, they had a hair dresser come in to give the patients a hair cut, my grandmother had very long hair, it hadn’t been brushed or washed since she arrived there which by this time was about 3.5 weeks and was in a matted ponytail so I went and bought a brush to try and detangle it before the haircut, I was able to do the top, but underneath was so matted that they had to cut it super short and at one point thought they were going to have to shave it. I am thankful for the front desk/administration office people who helped my mother with everything she needed, but thats where the good from this place ends. The CNA’s/nurses just seem like they don’t want to really be there and they seem like everything is just a huge inconvenience of their time, i understand their job is very, very difficult and stressful, but having a little bit of a friendly smile, or friendly disposition wouldn’t hurt any of them.

Dominic has been a fabulous caretaker for me during my stay here (and for anyone else I noticed that he’s taken care of very well as well.) He’s gentle and kindly and efficient. In all of things that I’ve seen or noticed him doing for me and for others actually. If I could hire a CNA I would hire Dominic to travel the world and see that I made it there safely!

Dominic has been extremely kind and sweet to Doris…the patient….he’s gone above and beyond our expectations….Doris can be difficult…..but Dominic greets her with a smile..treats her with loving care. Makes her feel special! He’s just outstanding! He’s Doris’s favorite! I also want to comment on nurse Tracy….shes sweet and personal. Overlooks being ignored. I’m Doris’s caregiver…..I understand her… The staff here allows me and her family to rest in knowing she’s in good hands. Numerous questions are answered politely, quickly. Doris’s needs are also met quickly, response time is remarkable. I’m here daily, the staff tells me her progress which I, in turn, relay to her family. The family, being out of state, can rest easy knowing she’s being cared for properly, with love. I’m impressed by the entire staff, from check in desk to cleaning staff. Top notch, truly top notch!! I’ve been treated with respect, my concerns are treated with the same respectful responses. The staff works as a team, with the goal to get Doris strong and well. Great staffing!

My mom was in the facility and Dominic was great and very attentive from the time she came her to the day she left. A majority of the Staff was very nice.

First of all the entire staff were attentive and gave me excellent care. The rehabilitation dept is excellent and better than other acute care facilities I have been in. Three individuals that gave me exceptional care were Nurses Tara and CNA Dominic. Nurse Tara was great. They were very knowledgeable about what was going on with me and they listened to my needs. As for Dominic.He is my superstar.When I first came in he made me feel welcomed and took away my anxiety. He listened I did not feel ignored. Any questions I had he answered without hesitation and if he didn’t have the answer he was quick to say I will find out for you. He was attentive to my needs and made my stay a great stay.

I had a wonderful experience at Trellis Paradise. From the moment we arrived, the staff made us feel comfortable and cared for. The nurses, CNAs, and therapists were all kind, patient, and professional. You can really see that they care about their patients and treat them with respect. The therapy team was amazing and very supportive throughout the recovery process. The facility itself is clean, organized, and has a welcoming environment. Even the food was good and well-prepared. Overall, I highly recommend Trellis Paradise to anyone looking for quality rehab and compassionate care. Thank you to everyone who helped make our stay a positive one!

Laiza the case manager is amazing. She is showing seriousness and caring for her patients and job. Very good experience because of her. THANK YOU Laiza 5 stars is just for you but, I wouldn’t say the same for some of the CNAs. But, Karina the nurse is 2 faces pretending to be Kind but she is not. Good actress Karina pretending so well thinking people are stupid and don’t see her acts. Karina, you will get old one day and Karma will meet you there.
