We are proud to announce that we’ve been recognized by the Centers for Medicaid and Medicare Services (CMS) as a Five-Star Overall and Quality Measures facility. Nursing homes are rated from 1-5 stars based on health inspections, staffing, and quality measures. According to CMS, a rating of five stars means that quality is “much above average.”

I stayed at Trellis Paradise from May 13, 2026 to June 26, 2026, and want to thank the nurses , CNAs, OTs, PTs and other staff for the care that I received. I especially am grateful to Nurses Jerrick-best IV nurse, Alise and Jeff who had to take and put in new IVs on my arms and very professional. Nurse Jerrick is firm and made sure I worked with the PTs and OTs. PTs and OTs helped me develop skills and my muscles as a BKA. CNAs Sandra – efficient and nice, Irene- watches and checks on patients, Dominic- efficient and friendly, Gorgeous-encouraging and inspiring words, Gwendolyn, Amelia, Rose Cruz – bubbly, and Jaeleen and the other nurses who were proficient and pleasant. APRN Jessica was friendly and professional. My family had pleasant visits with me and we enjoyed the dining area and courtyard. Thank you for helping in my recovery and assisting with my transition to home care.
I really enjoyed my stay here with the exception of your food. You really need a better cook!. I enjoyed my CNA’ : DOMNIC (aka- DOM) CNA ,HE WAS VERY NICE, PERFECTLY CARING WITH ALL PATIENCE from what I witnessed with other patients. He really has a Heart and he shows that he LOVES HIS WORK. I also have to give credit to the nurse: Jerick( day shift) he is very attentive and also shows he cares for his patient’s and I Love his sense of HUMOR.. I also want to give Daniel-RN.a thumbs up also. He always greets me with a smile everyday makes me feel happy to see me, I also, give Kyla CNA A big thumbs up. She is so sweet and very attentive, professional. and cares for her patient’s. Loved my OT, & PT THERAPISTS they were GREAT AND PROFESSIONAL and great sense of Humor.
Paradise facility is a clean, well-lit Post-Acute Care Rehabilitation Center. Much gratitude to the O.T.s, P.T.s, C.N.A.s, R.N.s that look after my loved one. I see it I the staff’s continence. I see the compassion. I hear their voice. Care. Reception always greets me with a warm welcome. Engaging visitors with compassion and professionalism. …and the star the fell from the sky, right into the reception chair, Brandy. Thank you Brandy for your encouragement and kind words.
